Preparing for your first appointment at The Sports Clinic

Before your visit, make a list of questions you’d like to ask during your first meeting and call our office ahead of time if you have any questions at (905) 829-2827 or email us at wpo@sportsclinic.ca

You will register at the front desk when you arrive at our office. We’ll ask for any paperwork you may have brought with you (i.e., referral, X-ray or MRI report, and other medical reports).

What to bring:

  • Bring a list of current medications, allergies, and any relevant past surgeries.
  • Be prepared to talk about your medical history, previous treatments, limitations you’re dealing with, and other relevant details.
  • Comfortable, loose-fitting clothing that allows the physical therapist to examine and treat the affected area. For example, if you have a knee issue, consider wearing shorts.

When to arrive:

  • Plan to arrive 10-15 minutes before your scheduled appointment to complete any necessary paperwork and settle in. This will also give you time to ask questions and address concerns before your session begins.

What to wear:

  • Wear comfortable, loose-fitting clothing that allows easy movement and access to the treated area.
  • Athletic wear or clothing that can be easily adjusted is ideal.
  • Avoid wearing tight or restrictive clothing, which may hinder your ability to move freely during the session.
  • Wear supportive, closed-toe shoes, such as sneakers, to ensure safety and comfort during the session.

If you wish to save time on your first visit, please complete your intake paperwork online or print the patient forms below. Be sure to bring them with you to your appointment. If you do not have time to print them ahead of time, you should arrive at least 15 minutes early to give yourself plenty of time to finish them before your appointment.
