Physiotherapy for ACL Tears: What To Expect

1. Acute Phase

During this phase, you’ll focus on managing pain and reducing swelling. If you’re recovering from surgery, you’ll also want to ensure the surgical site is healing properly.

While you don’t want to remain in total bedrest (early movement is crucial in recovery), you want to ensure you’re performing the correct movements that won’t risk reinjury. Short, assisted walks and gentle range-of-motion exercises will help kickstart your rehabilitation.

2. Early Rehabilitation

This phase focuses on restoring knee movement, although we’ll also start incorporating muscle-strengthening exercises. This phase is crucial for laying the foundation for more intense rehab.

We’ll show you exercises (such as heel slides) to improve knee mobility and stability.

3. Progressive Strengthening

This phase marks the beginning of more intense rehabilitation, emphasizing building muscle strength to further enhance knee stability. Your pain levels should have decreased significantly by this point, and we’ll start you on a progressive exercise program to build strength throughout your lower body.

We might start with bodyweight exercises, such as squats or steps, then move on to more intense resistance training. You can also begin light aerobic activities, like jogging or cycling.

4. Functional Training

This phase is about preparing you to return to your favorite activities! We’ll begin incorporating training for sports-specific movements to ensure your knees can handle the necessary physical demands.

This is also the phase where we’ll really emphasize improving balance, gait, and proprioception (your body’s awareness of itself in space).

5. Return to Activity

Finally, we’ll help you navigate a safe return to your everyday activities, including your chosen sport. We’ll provide you with a return-to-activity schedule to help you gradually increase your activity levels. We can also suggest any activity modifications so you can jump back in safely and quickly.
